What to Expect at Your Exam

It is necessary to perform a comprehensive ophthalmology evaluation at your first exam which will take about two hours. Please feel free to ask any questions and take notes as needed.

We do our very best to stay on schedule, run our clinic as efficiently as we can, and keep your waiting time to a minimum. To help us achieve that, we require 48 hours notice if you need to cancel your appointment and we reserve the right to cancel an appointment if you are more than 15 minutes late for your scheduled appointment time unless prior arrangements have been made. Please bring the following items with you:

  • A list of current medications, including dietary supplements or herbal remedies
  • Your medical records- you may have your previous eye doctor forward them in advance of your appointment
  • Completed Forms: Privacy Notice, Patient Information, Patient Health
    (If not submitting electronically, the above forms can be found in Printable Versions here)
  • Insurance information/cards or Medicare card
  • Referral card of from your primary care physician, if required by your insurance carrier (Medicaid/Title XIX)
  • Your current glasses and/or contact lenses
